Play Musical Chairs with I've Been Working on the Railroad or similar songs or use a collection of recorded sound effects (either homemade or purchased).
Try "Duck, Duck, Goose" as "Boxcar, Boxcar, Locomotive"
Pin the Smokestack on the Locomotive
Sing The Wheels on the Bus as "The Wheels on the Train"
Have an adult in an engineer costume (hickory striped overalls), or an entertainer, read a train story or lead games and songs.
Have hats, bandanas, train whistles, etc. and let the kids dress up.
Railroad Tag

As few as five can play but more is better. This is a version of tag where a group does the tagging rather than just one person. Choose two players to start the train. They join hands and chase the other players in order to tag one. When tagged, a player joins hands with them and becomes part of the train. The game is over when all the players are part of the train.

Provide train-themed stencils and stickers and construction paper for drawing, or to decorate cardboard box "stations" and boxcars.

Make costumes out of child size boxes.

Use an unsealed box for each child and remove flaps or tape them open and let children decorate. An adult should attach two lengths of 2" wide ribbon to one side of the box using duct tape or a heavy duty stapler. Measure the child and attach the ribbon to the opposite side to form suspenders. Now form a line and have a railroad parade!

Make and decorate an instant camera photo-sized photo frame out of cardstock or craft foam. Take an instant photo of each child with the birthday child to send home in their custom frames.

Arrange finger foods on rectagular serving trays in a straight line at the head of the trays place a large toy, an inflatable or anything that can serve as a locomotive.

Fill a bandanna with small toys, whistle, candies, etc. and tie closed at the four corners. For extra fun tie to dowel or well sanded stick.
Fill a mug or plastic cup with small toys, whistle, candies, etc
